How often should you load the tendon in rehab? Tendon response to load takes time, consider both short term and long term reaction to load. Look at the figure shown here (Magnusson et al, 2010). After acute heavy exercise in humans, net loss of collagen production was reduced for 24 - 36 hours after very heavy loads. Tendons need recovery to allow adaptation, and do not want to be loaded heavy every day! This may explain why some patients go on to develop tendinopathy. They do too much, too soon, without adequate rest. So what are the clinical implications of this? If recovery between trainings session are inadequate it might lead to further injury instead of recovery. Therapeutic loading should be of high magnitude and only performed every other or third day.
